MFHT Wellness Challenge - Registration!

If you are interested in participating in the Wellness Challenge, please complete this form. Please note that each participant must fill out a form.

Participant Information

* Those who have a Facebook account can join the MFHT Wellness Challenge Facebook group to receive challenge information and to have the opportunity to connect with other participants.

* If you don’t have a Facebook account, all challenge information will be sent to you via email.

Team Options

You can register as an individual and be placed on a team or you can register a team of up to five people. If your team doesn’t include five members, other participants may be added to your team.

Each team will need a team captain. Team captains will be responsible for collecting their team’s points, tracking their team’s progress and ensuring that everyone on the team is receiving the challenge-related information and resources.

Aim to participate in 1 health behaviour per day and work your way up.

Aim to participate in 2 or 3 health behaviours per day.

Aim to participate in 3 or 4 health behaviours per day.


As part of the MFHT’s health promotion communication strategy, there will be some PR and messaging through local media, email, and/or text message throughout the challenge (i.e. sharing information on the MFHT’s website and Facebook page, the Mercury, the radio etc.). Do you give us permission to share pictures and information about the challenge that may include you?
